Information regarding medical records can be obtained by calling our main office number: (936) 539-5000 or Houston Metro: (936) 441-0226.

Medical records are kept confidential. We will not release any of your medical information without first obtaining a signed authorization from you.

As referenced in Form A - Patient Registration and Medical Records Release Authorization, if we assist any patient with medical forms other than those related to the current visit, a $25 fee will be assessed.
